Form 19: Registration holders of partly paid shares

2. Fill in the form

Fill in the form. On the form it says who may sign the form.

3. Send the form

Send the signed form to a KVK postal address, together with a copy of a valid proof of identity of the person signing the form.

Note: This is only possible in the case of a public limited company (NV). If an NV issues partly paid shares, you include the details of the shareholder (except the signature), the shareholding, and the amount paid on Form 19.
