You use form 1 to register an eenmanszaak (sole proprietorship) in the Business Register.
Please note: when you fill in this English form digitally, the corresponding field in the Dutch form is filled in automatically. Sign, print, and send the completed Dutch form to KVK.
Register your eenmanszaak online
You can register an eenmanszaak online. To finalise it, you still have to visit KVK in person. In some cases you do not need to come to KVK. Then you can complete Form 1 and send it by post. Are you continuing an existing establishment or business? Then you must also complete form 1.
What is this form for?
You use form 1 to register an eenmanszaak in the Business Register. With a sole proprietorship, 1 person is the owner, even though more people may be employed in the business. The owner can only have 1 eenmanszaak. An eenmanszaak can have more than 1 establishment.Â
You can use Form 1 to:
- Report that you are taking over an existing business or establishment and continuing it as an eenmanszaak.Â
- Registering an eenmanszaak if you live on the Wadden Islands and therefore do not need to visit the KVK.
Help with filling in
Address
The following rules apply to addresses entered on form 1:
- the address must be listed in the Basic Registration of Addresses and Buildings (BAG).
- A residential address must also match the Personal Records Database (BRP) of the Dutch municipalities.
Use the BAG Viewer (in Dutch) to check whether the address in the BAG is correct. This includes the street name, house number, addition (such as 24a or 24b), or designated use (such as retail premises). If something is incorrect, you can report the error on the BAG Viewer website.
The Business Register is linked to the Personal Records Database (BRP) of the Dutch municipalities. If your residential address is not listed in the BRP or is incorrect, you must first have it corrected by your municipality.
If the address in the BAG or BRP is incorrect, KVK cannot register the eenmanszaak in the Business Register. Only send or bring form 1 to KVK once the correct details are listed in the BAG and BRP.
Dates
Is the founding date of the eenmanszaak more than 2 months ago? Then KVK needs proof of this. For example, a purchase invoice or accountant's statement.
The dates you enter on this form may also be in the future, but no more than 3 months. KVK will ensure that the registration is visible in the Business Register 1 week before the formation of the eenmanszaak. In case of a takeover, the registration is visible in the Business Register from the acquisition date if possible.
Appointment or send in by post?
Check whether you need to visit KVK or can send the form by post. The following questions will help you settle this.
Are you registering a new eenmanszaak and do not live on the Wadden Islands?
You register the eenmanszaak online. The owner must make an appointment to visit a KVK office. You do not need to fill in form 1.
Are you taking over an existing business or branch and are you not registered with KVK?
Then fill in form 1 and visit a KVK office.
Do you live on the Wadden Islands or are you taking over an existing eenmanszaak or establishment and are you registered with KVK?
Then you can send the documents to KVK by post
Do you have an appointment for another form?
Then bring all documents with you to your appointment, even if you are allowed to send them by mail.

What do you always need?
A copy of a valid identity document. Do you have an appointment? Then bring a valid ID with you to your appointment.
Are you sending the documents?
Then KVK needs form 1.
Is the incorporation date of the eenmanszaak more than 2 months ago?
Then KVK needs proof of this. For example, a purchase invoice or accountant's statement.
Is the eenmanszaak a franchisee?
Then KVK needs the franchise contract.
Does the eenmanszaak have more than 1 establishment?
Register each establishment using form 9.Â
Do you want to register an authorised representative or agent in the Business Register?
Report the officials or agent authorised to act on your behalf using form 13.
Have you taken over a company or business?
The ex-owner of the company or establishment must report this to KVK. For an eenmanszaak or establishment, they may use form 14. To report the end (and takeover) of a partnership (a VOF, CV, or maatschap), they can use form 17. To dissolve a legal entity (a BV, NV, or cooperative), they can use form 17A. You must bring the original form to KVK. KVK also needs a copy of the proof of identity of the person who signed the form,
Are you using a different address for your business or company than your home address?
Then bring proof that you are allowed to use the visiting and mailing address you provided for your eenmanszaak.
