Download: Form 1 Registration Eenmanszaak (sole proprietorship)

You use form 1 to register an eenmanszaak (sole proprietorship) in the Business Register. You can also use it to register a co-owner, in case the owner of the eenmanszaak passes away.

Please note: when you fill in this English form digitally, the corresponding field in the Dutch form is filled in automatically. Sign, print, and send the completed Dutch form to KVK.

What is this form for?

Only one person owns an eenmanszaak, even if several persons are employed. The owner can only have one eenmanszaak. An eenmanszaak can have more than one establishment. 

You can use form 1 to:

  • register a new eenmanszaak.
  • report the takeover of an existing business or branch office as an eenmanszaak. 
  • register a co-owner for an eenmanszaak. You do this if two or more persons have inherited an eenmanszaak. You register one heir with form 1. The other heirs you register using form 12. 

Help with filling in

Address

The following rules apply to addresses entered on form 1:

  • KVK may only enter addresses in the Business Register that are listed in the Basic Registration of Addresses and Buildings (BAG).
  • A residential address must also match the Personal Records Database (BRP) of the Dutch municipalities.

Use the BAG Viewer (in Dutch) to check whether the address in the BAG is correct. This includes the street name, house number, addition (such as 24a or 24b), or designated use (such as retail premises). If something is incorrect, you can report the error on the BAG Viewer website.

The Business Register is linked to the Personal Records Database (BRP) of the Dutch municipalities. If your residential address is not listed in the BRP or is incorrect, you must first have it corrected by your municipality. Otherwise, KVK cannot register the eenmanszaak in the Business Register.

Only send or bring form 1 to KVK once the correct details are listed in the BAG and BRP.

Dates

The dates you enter on this form may also be in the future. KVK will ensure that they are visible in the Business Register from that moment on.

Appointment or send in by post?

Check whether you need to visit KVK or can send the form by post. The following questions will help you settle this.

Has a civil-law notary signed this form?

If the civil-law notary signs and sends the documents to KVK, you do not need to do anything. 

Does the owner of the eenmanszaak sign form 1?

Then the owner must make an appointment to visit a KVK office.

Do you have an appointment with the KVK because of another form?

Then bring all documents with you to your appointment, even if you are allowed to send them by mail.

Make an appointment at kvk.nl/afspraak (in Dutch).

Which documents do you need to bring?

  • Valid ID (not a copy)
  • Form 1

Does the eenmanszaak have more than one office?

Register each branch office using form 9. Bring it to your appointment.

Do you want to register an authorised representative or agent in the Business Register?

Report the officials or agent authorised to act on your behalf using form 13.

Have you taken over a company or business?

The ex-owner of the company or branch must report this to KVK. For an eenmanszaak or branch office, they may use form 14. To report the end (and takeover) of a partnership (a VOF, CV, or maatschap), they can use form 17.  To dissolve a legal entity (a BV, NV, or cooperative), they can use form 17A.

You, the new owner, must bring this form to KVK. This must be the original form (no copy or scan). 

Are you using a different address for your business or company than your home address?

Then bring proof that you are allowed to use the visiting and mailing address you provided for your eenmanszaak.
