KVK forms for registration and reporting changes

Registering a business or reporting changes can be done online in most cases (in Dutch). Click on the buttons below to start the online process. Otherwise, you can download the relevant form and send it by post. Most KVK forms are also available in English. You can find links to all KVK forms on this page.
A few basics
- You can fill in a PDF form on your computer, but you cannot send it digitally.
- When you fill in an English form, the attached Dutch version is automatically filled in.
- After filling in the form on-screen, you must print it - including the attached Dutch form.
- Remember to also sign the Dutch version of the form.
- Please note that if you fill out free text fields, like for your business activities, you must provide a Dutch translation.
- The form pages state whether you can send it by post or must bring it in person.
- If you have to deliver the form in person, you need to make an appointment.
- If you send a form by post, you must always include a copy of a valid proof of identification of the person signing the form.
Forms to register a business, organisation, legal entity, or establishment
Form 1: Register an eenmanszaak (sole proprietorship)
You use Form 1 to register an eenmanszaak (sole proprietorship). You also use this form to report that an existing company has a new owner. After the owner's death, you can also use the form to register a new owner or co-owner of the sole proprietorship. You can also fill in this form online (only available in Dutch).
Form 2: Register a VOF, CV, or rederij
Use Form 2 to register a VOF (general partnership), CV (limited partnership), or rederij (shipping company). You can also fill in this form online (only available in Dutch).
Form 2a: Register a maatschap
You use Form 2a to register a maatschap (professional partnership). You can also fill in this form online (only available in Dutch).
Form 3: Register a BV or NV
With Form 3, you register a BV (private limited company) or NV (public limited company).
Form 4: Register a Coöperatie or onderlinge waarborgmaatschappij
Form 4 is used to register a coöperatie (cooperative) or onderlinge waarborgmaatschappij (mutual insurance association).
Form 5: Register a Stichting, vereniging, or VvE
With Form 5, you register a stichting (foundation), vereniging (association), or VvE (owners' association. You can also use this form to report that an informal vereniging has changed into a formal vereniging.
Form 6: Register a Dutch establishment of a foreign partnership or legal entity
With Form 6, you register a foreign legal entity or foreign partnership.
Form 7: Register a BV or NV in oprichting
With Form 7, you register a BV (private limited company) or NV (public limited company) in oprichting (in the proces of incorporation).
Form 8: Register the business of a legal entity
With Form 8, you register a business of a legal entity.
Form 9: Register an establishment of a business
With Form 9, you register an establishment (business location) of a company. You can also do this (only available in Dutch).
Form 9a: Register an establishment of a vereniging, stichting, or legal entity under public law without a business
With Form 9a, you register an establishment (business location) of a vereniging (association), stichting (foundation) or public law legal entity. You can also do this (only available in Dutch).
Form 20: Register a European Economic Interest Grouping
You use Form 20 to register a European Economic Interest Grouping (EEIG).
Form 23: Register a European Company
Use Form 23 to register a European Company (SE). You also use this form to report that you are continuing an existing company in the form of an SE.
Form 24: Register a European Cooperative Society
You use Form 24 to register a European Cooperative Society (SCE). You also use this form to pass on that you are continuing an existing company in the form of an SCE.
Form 25: Register a legal entity under public law
With Form 25, you register a publiekpersoon (legal entity under public law) in the Business Register. Such an entity carries out government functions.
Form 27: Register a kerkgenootschap
You use Form 27 to register a religious organisation with the legal form kerkgenootschap in the Business Register. A religious organisation is an umbrella organisation, a kerkgenootschap (religious association), or an independent religious body such as a school, convent, or mission.
Form 28: Register other legal entities under private law
With Form 28, you register a private legal entity such as a boermarke, fundatie, buurschap, or gilde. If you want to register another private legal entity for which there is no form, please contact KVK.
Form 18: Add or change information for other forms
Use Form 18 if you do not have enough space on a KVK registration or change form to fill in the requested details.
Forms to register an official or representative
Form 10: Register partners of a VOF, CV, or rederij
Use Form 10 to register one or more vennoten (partners) of a VOF (general partnership), CV (limited partnership), or rederij.
Form 10a: Register partners of a maatschap
Use Form 10a to register one or more maten (partners) of a maatschap (professional partnership).
Form 11: Register officials for a legal entity
With Form 11, you register one or more directors, supervisory directors, trustees, or an only shareholder for a legal entity.
Form 12: Register officials for a BV i.o., NV i.o, or eenmanszaak
With Form 12, you register an official who is authorised to act on behalf of a BV (private limited company) or NV (public limited company) in oprichting (in the process of incorporation). You can also use this form to register a co-owner of an eenmanszaak.
Form 13: Register an authorised representative or business agent
With Form 13, you register one authorised representative or businessagent. The authorised representative may perform certain acts on behalf of a company or legal entity. Print the form and send it by post. You can also register an authorised representative for a stichting (foundation) or vereniging (association) (only available in Dutch).
Form 19: Register holders of partly paid-up shares
Form 19 is used to register one or more holders of partly paid-up shares of an NV (public limited company), a European Company (SE), or a European Cooperative Society (SCE). A holder of partly paid shares can be either a natural person or a legal entity (such as a BV, stichting, or vereniging).
Form 21: Register officials for a European Economic Interest Grouping
With Form 21, you register one or more officials of a European Economic Interest Grouping (EESV). An official can be either a natural person or a partnership or legal entity. You can also register a director (only available in Dutch).
Form 22: Register officials for a stichting or vereniging without a business
With Form 22, you register officials for a stichting (foundation) or a vereniging (association) without a business or a VvE (association of owners) in the Business Register. This can also be done (only available in Dutch).
Form 26: Register officials for a legal entity under public law with a business
Use Form 26 to register officials of a managing or supervisory body for a publiekrechtelijk rechtspersoon met onderneming (legal entity governed by public law with a business).
Form 29: Register officials for a boerenmarke, hofje, gilde, buurschap, kerspel, or fundatie
You can use Form 29 to register members of the governing body and members of the supervisory body of a boerenmarke, hofje, gilde, buurtschap, kerspel, or fundatie. An official can be a natural person, partnership, or legal entity

Use Form 18 if you do not have enough space on a KVK registration or change form to fill in the requested details.
Forms to report a change for a business, organisation, establishment, or official
Form 14: Change the details of a business or establishment
Use Form 14 to change details of a business or establishment (business location) in the Business Register. This can also be done (only available in Dutch). Use this form to report that an eenmanszaak (sole proprietorship) or branch office has been transferred or ended.
Form 15: Change the details of a legal entity or partnership
With Form 15, you report changes for a legal entity or business. For example, if the amount of capital or the articles of association have changed. You can use this form for a VOF (general partnership), maatschap (professional partnership), CV (limited partnership, BV (private limited company, NV (public limited company), coöperatie (cooperative), onderlinge waarborgmaatschappij (mutual insurance association), stichting (foundation), vereniging (association), owners' association (VvE), foreign legal entity, legal entity under public law, or religious organisation.
Form 16: Change details of officials or deregister officials
With Form 16 you change the details of officials or deregister officials. An official can be a vennoot/maat (partner), sole shareholder, director, supervisory director, or authorised representative. You can also do this in My KVK in some cases.
Form 18: Add or change information for other forms
Use Form 18 if you do not have enough space on a KVK registration or change form to fill in the requested details.
Forms to deregister a company or legal entity
These forms allow you to dissolve a company or legal entity. Dissolving means that the company or legal entity stops all activities. But the legal entity continues to exist to settle the remaining assets and debts.
Form 14: Change the details of a business or establishment
Use Form 14 to change details of a business or establishment (business location) in the Business Register. This can also be done (only available in Dutch). Use this form to report that an eenmanszaak (sole proprietorship) or branch office has been transferred or ended.
Form 17: Reporting a dissolution of a VOF. CV, maatschap, or rederij
With Form 17, you report that a VOF (general partnership), CV (limited partnership, maatschap (professional partnership) or rederij (shipping company) has been dissolved.
Form 17a: Reporting a dissolution of a legal entity
With Form 17a, you report that a legal entity has been dissolved. A legal entity is, for example, a BV (private limited company), coöoperatie (cooperative), onderlinge waarborgmaatschappij (mutual insurance association), stichting (foundation), or vereniging (association). Does the legal entity no longer have any income (assets) at the time of dissolution? Then you can also fill in the help form for fast-track liquidation.
Form 17b: Reporting the end of a legal entity after liquidation
With Form 17b, you report that a legal entity no longer exists. This requires that the legal entity has been dissolved and the assets and debts have been liquidated. You must have previously reported the dissolution to the KVK with Form 17a.

This form will help you collect documents needed when ending a legal entity with fast-track liquidation. This is only possible if the legal entity has no assets.
UBO registration forms
Use the forms below to register your organisation's UBOs (ultimate beneficial owners). Is your company in the Business Register? Then register your UBO's online, but be aware: most of the procedure is in Dutch. To register UBO's for a new company or legal entity use one of the forms below.
For an NV, BV, SE, or SCE
Form 30: UBO registration for a new NV, BV, SE, or SCE
Use form 30 to register a UBO for a BV (private limited company), NV (public limited company), European public limited company (SE), or European cooperative society (SCE).
For a stichting
Form 31: UBO registration for a stichting
With Form 31, you register a UBO for a stichting (foundation).
For a CV, VOF, maatschap, EESV, or rederij
Form 32: UBO registration for a CV, VOF, maatschap, EESV, or rederij
Use Form 32 to register one or more UBOs of a VOF (general partnership), CV (limited partnership), maatschap (professional partnership), European Economic Interest Grouping (EEIG), or rederij (shipping company). Is there no evidence of the UBO's interest in the VOF, CV, maatschap, EEIG, or rederij? Then you must also complete a UBO document.
For a vereniging, coöperatie, or onderlinge waarborgmaatschappij

Use Form 33 to register a UBO for a vereniging (association), coöperatie (cooperative), or onderlinge waarborgmaatschappij (mutual insurance association).
For a religious organisation
Form 34: UBO registration for a religious organisation
Use Form 34 to register a UBO for a religious organisation.
Frequently asked questions
If you can post the form to us or if you must deliver it in person depends on the situation. The instructions on the form will tell you whether you can post it or not. Somettimes you must identify yourself in person at one of our offices. You can notify us of many changes online.
If you must visit us in person, bring the completed and signed form with you. You can only visit by appointment. Log in with Digid to make your appointment online. You can register at any KVK office in the Netherlands.
In busy periods it can be difficult to make an appointment. So plan ahead when booking an appointment. Bear in mind that you may have to wait more than a week for an appointment.
How quickly we process your change or deregistration depends on how you communicate it to us: online or by post. Check the current processing times on our website. Follow the progress of your online change or deregistration in My KVK.
The change or deregistration is not correct
If the information you provide is not correct, we will contact you.
Changes or deregistration for a date in the future
Are you submitting a change or deregistration for a future date? Then you will only see it in the Business Register on that date. For the UBO register, you cannot submit changes with an effective date in the future.
Confirmation by letter
You usually receive a confirmation letter when all changes have been processed.
Digital confirmation
Do you have a Message for businesses? Then you will receive the letter from the Business Register there.
Do you have a Message Box (for private individuals)? Then you will receive the letter from the UBO register there.
This is only the letter about your personal UBO registration, change, or deregistration.
Confirmation by email
Do you make the change online? Then you will also receive a confirmation via email.
You do not need to register separately with the Netherlands Tax Administration. After registration, we will pass this information on. You will receive a notification from the Tax Administration by post within 2 weeks of the start date of your business.
