What is this form for?

With form 26 you can register 2 types of officials in the Business Register:

• Members of a governing body of the public-law legal entity with a business.
• Members of a supervisory body of the legal entity under public law with a business.

A legal entity under public law performs government tasks.

Appointment or send in by post?

Do you not have an appointment with KVK for another form?

Then send form 26 and the documents to KVK by post.

Do you have an appointment with KVK for another form?

Then bring form 26 and any other documents needed to the appointment with KVK.

What do you always need to send?

• A copy of a valid ID of the member of the governing body who signed Form 26. Make the photograph unrecognisable. If you have an appointment with KVK, bring a valid proof of identity.
• Form 26.

Do you want to register more than 7 members of the governing body or 7 members of the supervisory body?

Then use another form 26. Or, you can also enclose a document showing all members of the managing body and supervisory body. For each member, you must list the details as requested on form 26. The document must be authenticated by a civil-law notary.

Is the legal entity under public law not yet registered?

Use form 25 to register the legal entity under public law. A legal entity under public law must always appoint a private authorised representative. Please use form 18 and state the name or position of the authorised person under private law. Also record in writing the contents of the authorisation.

Do you want to register the business of the legal entity under public law?

Use form 8.

Does the legal entity under public law have establishments with a business?

Then register each establishment using form 9.

Does the legal entity governed by public law have establishments without a business?

Register each establishment with form 9a.
