Form 30: Register UBOs for a BV, NV, European Company, or European Cooperative Society

You use Form 30 to register ultimate beneficial owners (UBOs) of a private limited company (BV) in the Business Register. You can also use this form to register UBOs of a public limited company (NV), European company (SE), or European cooperative society (SCE).

Please note: when you fill in this English form digitally, the corresponding field in the Dutch form is filled in automatically. Sign, print, and send the completed Dutch form to KVK.

If the BV, NV, SE, or SCE is already registered in the Business Register, you can register the UBOs online (only available in Dutch).

Why use this form?

On form 30, you fill in what interests a UBO has in the BV, NV, SE, or SCE. Under European legislation and the Business Register Act, all BVs, NVs, SEs, and SCEs must register UBOs.

Ultimate beneficial owners (UBOs) are the ultimate owners of an organisation or persons who make the final decisions in an organisation. A UBO is always a natural person: a human being. Read how to determine who is the UBO of a BV, NV, SE, or SCE.

KVK will enter the information you provide on form 30 in the UBO register. This register helps to combat money laundering, fraud, and the financing of terrorism .

Help with filling in

The dates you enter on this form may also be in the future. KVK will ensure that they are visible in the Business Register from that moment on.

UBO legislation came into force on 27 September 2020. If a person already met the conditions to be a UBO of the BV, NV, SE, or SCE before that date, enter 27 September 2020 on form 30 as the date on which the person became the UBO.

Has a civil-law notary signed this form?

If the civil-law notary signs and sends the documents to KVK, you do not need to do anything.

Does a director or authorised representative sign this form?

Then send form 30 and the other documents to KVK by post.

Which documents do you need to bring or send?

Check which documents you need to send or bring to KVK. The questions below will help you with this.

What do you always need to send?

  • A valid proof of identity (ID) of the person who has an appointment at KVK. Or a copy of the ID of the person who has signed form 30. The copy must be in colour, A4 size and of both sides of the identity document. For a passport, KVK needs a copy of the page with the name and the page with the citizen service number (BSN). Make sure the photo is unrecognisable.

Is a person a UBO because they are a shareholder, hold voting rights, or have an economic interest?

Then KVK needs proof of the UBO's interest in the BV, NV, SE, or SCE. This proof must state the size of the interest.

Is a person a UBO because they are the person having effective control?

Then KVK needs proof that the UBO can make important decisions independently.

Is a person a UBO because they are a managing director or sole shareholder?

Then that person must be registered in the Business Register as a statutory (managing) director or sole shareholder. If this is not the case, you must register the managing director or sole shareholder in the Business Register.

Do you want to shield the details of a UBO in the register because the UBO is under guardianship or in receivership?

Then KVK needs an extract from the Central Guardianship and Administration Register showing that the UBO is under guardianship or in receivership. You can download this extract from rechtspraak.nl (in Dutch).

Do you want to shield the details of a UBO in the register because the UBO is under police protection?

KVK then needs the registration number of the Surveillance and Protection System (in Dutch) as proof that the UBO is receiving police protection. Does the UBO not have a registration number? Then KVK is not allowed to shield the details on the grounds of police protection.

Are you registering officials, a sole shareholder, or a BV, NV, SE, or SCE as well as 1 or more UBO?

Then you must register them using other forms. Combine form 30 and all other documents.

Does a UBO not live in the Netherlands, but inside the European Union?

In that case, KVK requires an extract from the population register in that EU country. The extract must not be more than two months old. If the document is not in Dutch, English, German, or French, please request an extract and a ‘multilingual model form’ from the population register. That is a translation that the population register attaches to the extract. Check the Netherlands Worldwide website to see what requirements apply for each country.

Does a UBO live outside the European Union? 

In that case, KVK requires an extract from the foreign population register. The extract must not be more than two months old. In almost all cases, the extract must also be legalised. If the document is not in Dutch, English, German, or French, a sworn translator must translate the extract. The translation must also be legalised, unless the document has been translated in the Netherlands. KVK needs both the translation and the original extract.
