You use Form 32 to register the ultimate beneficial owners (UBOs) for a VOF, CV, maatschap, EEIG, or rederij.

Please note: when you fill in this English form digitally, the corresponding field in the Dutch form is filled in automatically. Sign, print, and send the completed Dutch form to KVK.

If the VOF (general partnership), CV (limited partnership), maatschap (professional partnership), European Economic Interest Grouping (EEIG), or rederij (shipping company) is already registered in the Business Register, you can register the UBOs online (only available in Dutch).

What is this form for?

On form 32, you fill in what interests a UBO has in the VOF, CV, maatschap, EEIG, or rederij. Under European legislation and the Business Register Act, a VOF, CV, maatschap, EEIG, or rederij. must register UBOs.

Ultimate beneficial owners (UBOs) are the ultimate owners of an organisation or persons who make the final decisions in an organisation. A UBO is always a natural person: a human being. Read how to determine who is the UBO of a VOF, CV, maatschap, EEIG, or rederij.

KVK will enter the information you provide on form 32 in the UBO register. This register helps to combat money laundering, fraud, and the financing of terrorism. 

Help with filling in

The dates you enter on this form may also be in the future. KVK will ensure that they are visible in the Business Register from that moment on.

UBO legislation came into force on 27 September 2020. If a person already met the conditions to be a UBO of the VOF, CV, maatschap, EEIG, or rederij before that date, enter 27 September 2020 on form 32 as the date on which the person became the UBO.

Has a civil-law notary signed this form?

If the civil-law notary signs and sends the documents to KVK, you do not need to do anything.

Does a partner in the VOF or maatschap, managing partner of the CV, director of the EEIG, member of the rederij, or authorised representative sign this form?

Then send form 32 and the other documents to KVK by post. 

Do you need to visit a KVK office for another form?

If so, please bring all the documents you need to send, including form 32, with you to that appointment.

Which documents do you need to bring or send?

Check which documents you need to send or bring to KVK. The questions below will help you with this.

What do you always need to send?

  • A valid proof of identity (ID) of the person who has an appointment at KVK. Or a copy of the ID of the person who has signed form 32. The copy must be A4 size and of both sides of the identity document. For a passport, KVK needs a copy of the page with the name and the page with the citizen service number (BSN). Make sure the photo is unrecognisable. 
  • A copy of the ID of each UBO named on the form. The copy must be in colour, A4 size, and of both sides of the identity document. For a passport, KVK needs a copy of the page with the name of the UBO and the page with the BSN of the UBO. Make sure the photos are unrecognisable. 
  • Form 32

Is a person a UBO because they hold voting rights or have an economic interest?

Then KVK needs proof of the UBO's interest in the OF, CV, maatschap, EEIG, or rederij. This proof must state the size of the interest.

Is a person a UBO because they are the person having effective control?

Then KVK needs proof that the UBO can make important decisions independently.

Is a person a UBO because they are a managing director?

Then that person must be registered in the Business Register as a statutory (managing) director. If this is not the case, you must register the managing director in the Business Register.

Do you want to shield the details of a UBO in the register because the UBO is under guardianship or in receivership?

Then KVK needs an extract from the Central Guardianship and Administration Register showing that the UBO is under guardianship or in receivership. You can download this extract from rechtspraak.nl (in Dutch).

Do you want to shield the details of a UBO in the register because the UBO is under police protection?

KVK then needs the registration number of the Surveillance and Protection System (in Dutch) as proof that the UBO is receiving police protection. Does the UBO not have a registration number? Then KVK is not allowed to shield the details on the grounds of police protection.

Are you registering officials of a VOF, CV, maatschap, EEIG, or rederij as well as 1 or more UBOs?

Then you must register them using other forms. Combine form 32 and all other documents.

Does a UBO live abroad?

Then KVK needs proof of the UBO's foreign residential address.

What is proof of a foreign residential address?

Proof of a foreign residential address abroad can be an original extract from the municipality's foreign population register. Or a legalised copy. A legalised copy from an official agency or bank showing the residential address can also be proof.

What is a legalised copy of a document?

A copy of a document is legalised by a stamp from, for example, a civil-law notary, consulate, embassy, or municipality.

What are the requirements for a foreign document?

The proof must not be older than 1 month. The document must be in Dutch, English, German, or French. If this is not the case, you must have the document translated. KVK will then need the translation and the original document.
