What is this form for?
You can use form 4 to:
- register a new cooperative or mutual insurance association
- report that you are taking over an existing organisation or branch and continuing it as a cooperative or mutual insurance association
Help with filling in
Address
The following rules apply to addresses entered on form 4:
- The address must be listed in the Basic Registration of Addresses and Buildings (BAG).
- A residential address must also match the Personal Records Database (BRP) of the Dutch municipalities.
Use the BAG (in Dutch) to check whether the address of the company or legal person is correct in the BAG. This includes the street name, house number, addition (such as 24a or 24b), or designated use (such as retail premises). If something is incorrect, you can report the error on the BAG Viewer website.
The Business Register is linked to the Personal Records (BRP) of the Dutch municipalities. If a residential address is not listed in the BRP or is incorrect, you must first have it corrected by your municipality.Â
If the address in the BAG or BRP is not correct, KVK cannot enter the cooperative or mutual insurance association’s registration in the Business Register. Only send form 4 to KVK once the correct details are listed in the BAG and BRP.
Dates
The dates you enter on this form may also be in the future. KVK will ensure that they are visible in the Business Register from that moment on.
Appointment or send in by post?
Check whether you need to visit KVK or can send the documents by post. The following questions will help you settle this.
Has a civil-law notary signed this form?
If the civil-law notary signs and sends the documents to KVK, you do not need to do anything.
Does a director or authorised representative sign this form?
Then the director or authorised representative must make an appointment to visit a KVK office.

Which documents do you need to send?
Check which documents you need to send to KVK to register the cooperative or mutual insurance association. The questions below will help you with this.
What do you always need to send?
- A valid proof of identity
- Form 4
Does the cooperative or mutual insurance association not use the home address of a director as its visiting or postal address?
If so, KVK requires proof showing that the cooperative or mutual insurance association is allowed to use the specified visiting and postal address.
Does the cooperative have limited or statutory liability?
If so, bring a list of members signed by the board. This should not be a copy or scan.
Do you want to register managing or supervisory directors for the cooperative or mutual insurance association?
Use form 11 and bring this with ypu.
Is a (managing) director an ulitmate beneficial owner (UBO)?
If so, report this using form 33. You must declare UBOs to KVK. If you do not provide any UBOs, KVK cannot register the cooperative or mutual insurance association.
Do you want to register an authorised person or authorised commercial agent for the cooperative or mutual insurance association in the Business Register?
Register that authorised person or authorised commercial agent with form 13.
Has the cooperative or mutual insurance association taken over a branch or company?
The ex-owner of the company or branch must report this to KVK. For an eenmanszaak or branch office, they may use form 14. To report the end and takeover of a partnership (a VOF, CV, or maatschap), they can use form 17. To dissolve a legal entity (a BV, NV, or cooperative), they can use form 17a. KVK needs the original form (no copy or scan).
Does the cooperative or mutual insurance association have more than one branch?
Register each branch separately using form 9.
